Attic intake sealed off by paint or blown insulation while exhaust sits uselessly at the ridge, kickout flashings left off where a garage roof meets a two story wall, and vaulted rooms with no air channel above the insulation producing an ice ridge in the same spot every winter. All three are inexpensive to correct while a roof is already off, and all three are routinely skipped.
